Care Managers

Care Managers coordinate with a network of local service providers to secure access to programs, services, and activities for families and their children. These services are comprehensive, integrated, and provide along a Continuum of Care consisting of three levels: prevention, intervention, and intensive intervention. The Care Management Program will assist families in navigating the service delivery system along this continuum.

The Care Management program is thoughtfully designed and guided by the following principles to support the healthy development of children and their families:

  • Building on family and community assets
  • Voluntary family participation
  • Family focus and decision making
  • Inclusive and cultural competency
  • Collaboration
  • Coordination of existing services
